Before attending the seminar, we would like you to register through our Medical Software. This will consist of your demographic information and medical history. Remember the more information we have before your surgeon consult the less paperwork you will have to fill out on the day of your Initial visit. Please try to be as accurate as possible when entering your information. Please click on the Exemplo Icon below to start your registration. The User Id is the same for everyone: innova. The password will be your Social Security number. It is very important that you do not change or alter your User ID or Password .This will create duplicate records. The clinic staff does not have access to view your password.
There are consent forms that are not available through the medical software that must be signed on the day of your Initial Surgeon Consult. You can print these consent forms and bring them with you to your visit or they will be provided for you at your Initial Consult.
Our Bariatric clinic understands that not everyone will be able to complete the medical software registration. If you are unable to complete the registration, please feel free to contact us.
