Before your first appointment, we would like you to register through our Medical Software. This will consist of your demographic information and medical history. The more information we have before your surgeon consult the less paperwork you will have to fill out on the day of your Initial visit. Please try to be as accurate as possible when entering your information. Please click on the Exemplo Icon below to start your registration. The User Id is the same for everyone: innova. The password will be your Social Security number. It is very important that you do not change or alter your User ID or Password. This will create duplicate records. The clinic staff does not have access to view your password.

here are consent forms that are not available through the medical software that must be signed on the day of your Initial Surgeon Consult. You can print these consent forms and bring them with you to your visit or they will be provided for you at your Initial Consult. If you are unable to complete the registration, please feel free to contact us.

Patients are encouraged to print out and complete the following forms, and bring them to the hospital on the day of admission. This will help facilitate your admission process and significantly reduce your waiting time.
